About Guest Dosing
If you are 25 miles or more from your home Methadone-clinic, Maryhaven can offer guest dosing for up to 14 days (or longer with staff approval). Cost is $15 per day (cash only at this time). If you seek residential services with us, you may be eligible to guest-dose for up to 30 days as an in-patient.
To guest dose, fax the following information to our OTP clinic at 614/545-0238:
- A signed release of information form
- A signed order from your home treatment provider that specifically identifies your current dose and the dates you wish to guest-dose
- Three months of urine drug screens and dosing history
Note: A lock-box is required to receive take-home doses for Sundays.
Dosing is available at 1791 Alum Creek Dr., Columbus, OH 43207
- Monday-Friday from 6-11 a.m.
- Saturday from 8-10 a.m.
- Closed Sunday
About Transfers
Should you choose to transfer your care to us from another OTP provider, please fax the following information to our OTP clinic at 614/545-0238.
- A signed release of information form
- Current medication order signed by program physician
- Six months of urine drug screens and dosing history
- History & Physical Exam if completed within last 6 months
- Lab test results, including bloodwork and TB test results
All information will be reviewed by our program physician for approval for transfer.
