Maryhaven prioritizes the privacy and security of your information. This notice serves to inform you of a recent data security incident that resulted in the unauthorized acquisition of your protected health information (PHI) and/or personally identifiable information (PII). Maryhaven provides residential and outpatient medical services, including peer-to-peer step recovery programs. This notice is intended to provide you details about the incident, our response, and additional steps you may take protect your information. We were unable to locate all the addresses of the individuals whose information was involved. As a result, we are issuing this public notice to inform all potentially impacted individuals.
The review of PHI and PII potentially involved is near complete. Maryhaven will mail written letters on a rolling basis to potentially impacted individuals for whom Maryhaven has a sufficient address.
What Happened?
On June 1, 2024, Maryhaven discovered unauthorized activity in its network and, took immediate action to investigate the incident, including retaining independent forensics specialists. After a thorough investigation, we learned that an unauthorized actor gained access to our systems on May 30, 2024. On February 12, 2025, after a detailed review of the data involved, it was determined that the unauthorized actor acquired protected health information and personally identifiable information.
What Information Was Involved?
The information that may have been involved will not be the same for every potentially impacted individual. The information involved may include contact information (such as first and last name, address, phone number, and email), date of birth, and one or more of the following: driver’s license, medical diagnosis or condition, Social Security number, and health insurance information.
What We Are Doing:
We reported the incident to law enforcement and the Department of Health and Human Services Office of Civil Rights.
For those individuals who had their Social Security number impacted, we are offering access to single bureau credit monitoring services at no cost. This service provides alerts for 12 months from the date of enrollment when changes occur to the enrollee’s credit file. In addition, a dedicated team and call center will be available to provide support services and address inquiries concerning the incident, credit monitoring, identity protection services, and proactive fraud assistance to help with any questions that those individuals might have.
These services will be provided by Cyberscout, a TransUnion company, specializing in fraud assistance and remediation services.
If an individual is not sure whether their Social Security number was involved in the incident, they may call our hotline at 1-800-405-6108, and a specialist will assist them. If it is determined that their Social Security number was not involved in the incident, they will not be eligible for credit monitoring services.
What You Can Do:
To enroll in credit monitoring services, individuals may call our hotline at 1-800-405-6108 and ask to enroll. Individuals must verify their identity to confirm their eligibility for the credit monitoring services. For these services to go in effect, individuals must enroll, and the monitoring included in the membership must be activated to be effective. Please note that credit monitoring services may not be available for individuals who have not established credit or an address in the United States (or its territories), a valid Social Security number, or it was determined that your Social Security number was not involved in the incident. Enrolling in this service will not affect credit scores. If individuals need assistance, Cyberscout will be able to do so.
To date, we are not aware of any reports of identity fraud or fraudulent activity involving your information as a result of this incident. However, it is always prudent for persons to remain vigilant against incidents of identity theft and fraud by reviewing your credit reports and account statements for suspicious activity and to detect errors. If you discover any suspicious or unusual activity on your accounts, please promptly contact the financial institution or company. We have provided additional information below, which contains more information about steps you can take to help protect yourself against fraud and identity theft. Please also refer to this FAQ.

STEPS YOU CAN TAKE TO HELP PROTECT YOUR INFORMATION
Monitor Your Accounts and Credit Reports
We encourage you to remain vigilant against incidents of identity theft and fraud by reviewing your credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Under U.S. law, you are entitled to one free credit report annually from each of the three major credit reporting bureaus, TransUnion, Experian, and Equifax. To order your free credit report, visit www.annualcreditreport.com or call 1-877-322-8228. Once you receive your credit report, review it for discrepancies and identify any accounts you did not open or inquiries from creditors that you did not authorize. If you have questions or notice incorrect information, contact the credit reporting bureau.
You have the right to place an initial or extended “fraud alert” on a credit file at no cost. An initial fraud alert is a one-year alert that is placed on a consumer’s credit file. Upon seeing a fraud alert, a business is required to take steps to verify the consumer’s identity before extending new credit. If you are a victim of identity theft, you are entitled to an extended fraud alert lasting seven years. Should you wish to place a fraud alert, please contact any of the three credit reporting bureaus listed below.
As an alternative to a fraud alert, you have the right to place a “credit freeze” on a credit report, which will prohibit a credit bureau from releasing information in the credit report without your express authorization. The credit freeze is designed to prevent credit, loans, and services from being approved in your name without your consent. However, you should be aware that using a credit freeze may delay, interfere with, or prohibit the timely approval of any subsequent request or application you make regarding a new loan, credit, mortgage, or any other account involving the extension of credit. Pursuant to federal law, you cannot be charged to place or lift a credit freeze on your credit report. To request a credit freeze, you should provide the following information:
- Full name (including middle initial as well as Jr., Sr., III, etc.);
- Social Security number;
- Date of birth;
- Address for the prior two to five years;
- Proof of current address, such as a current utility or telephone bill;
- A legible photocopy of a government-issued identification card (e.g., state driver’s license or identification card); and
- A copy of either the police report, investigative report, or complaint to a law enforcement agency concerning identity theft, if you are a victim of identity theft.

You can further educate yourself regarding identity theft, fraud alerts, credit freezes, and the steps you can take to protect your personal information by contacting the credit reporting bureaus, the Federal Trade Commission (FTC), or your state Attorney General. You have the right to file a police report if you ever experience identity theft or fraud. Please note that in order to file a report with law enforcement for identity theft, you will likely need to provide some proof that you have been a victim. Instances of known or suspected identity theft should also be reported to law enforcement, your state Attorney General, and the FTC.
This notice has not been delayed by law enforcement.
